Health and Welfare Medical Benefits

Active Participant health care benefits provided by the Fund are administered through Cigna Health and Life Insurance Company (CIGNA).

Provider network directories are available upon request at the Fund office at (216) 781-6858 or (800) 342-7020 or by calling Cigna at (855) 444-4985 or you may visit Cigna’s website at www.cigna.com or download Cigna's mobile App for Android or Apple for an up-to-date network Provider listing.

Your Identification Card contains all the necessary information needed by you and your health care Providers to verify eligibility and benefits, and for submitting claims. Carry your card with you at all times; you must present this card to your healthcare Provider in order to receive benefits for claims on Covered Services.

You will receive one Identification Card for each Eligible Dependent on the Plan. Please note, however, that all cards must bear the insured’s name and/or your Eligible Dependent’s name and your 9-digit identification number followed by a two-digit person code (01, 02, 03 etc.). You may contact the Fund office or Cigna to obtain additional Identification Cards.

Receipt or possession of an Identification Card does not guarantee benefits under this Plan. Remember: eligibility, enrollment and contribution requirements must be satisfied before you are eligible to receive benefits.

In addition to the Cigna provider network, Active Participants and their eligible Covered Eligible Dependents enrolled in one of the Plans offered by the Cleveland Bakers and Teamsters Health and Welfare Fund will have direct access to Paladina Health. Paladina Health provides direct primary care medical services to Participants and their Eligible Dependents via on-site/or near-site Physician’s offices. Primary care services performed at the Paladina Health clinic will be covered at 100% with no cost share to the Participant. Pre-Medicare Retired Participant health care benefits provided by the Fund are administered through Cigna Health and Life Insurance Company (CIGNA).

Provider network directories are available upon request at Humana at (216) 328-2047 or (877) 310-5386. You may also visit Humana’s website at www.humana.com.

Carry your card with you at all times; you must present this card to your healthcare Provider in order to receive benefits for claims on Covered Services.
